Thank you for your interest in updating your ATI Communication Preferences. Please print out the following permission form and have your parent/guardian fill it out. Then send it to the address or fax number provided on the form.

Please ensure your browser is set to print on letter size paper. The permission form will not print correctly on any other size paper.

When we receive your signed permission form we will email you a confirmation of receipt and your communication preferences.
