ATI Parent/Guardian Permission Form Overview
Signing up for your ATI Communication Preference Center is
the easiest way to take advantage of everything ATI has to
offer on our website(s). Be sure to keep an eye on ati.com
for all of the latest activities and promotions!
As part of the registration process, we require certain information
from each participant as listed on the following form. Before
collecting this information from children 12 and under, we
require parent/guardian permission. To grant your child permission
to register for your ATI Communication Preference Center, please
complete the permission form and send it to the address provided
at the bottom of the form.
For this program, we require that the e-mail addresses for
parents/guardians and their children be different. This is
because once we process the permission form, your child will
receive an e-mail with a temporary user name and password that
will allow your child to log in to the website to complete
their registration. As the child's parent/guardian, you will
also receive an e-mail with a user name and password which
will allow you to change or update the access your child has
to the various activities on www.ATI.com.
There are many ways to participate on ati.com. Online activities
include participation in sweepstakes, surveys and product registration.
In both cases, parental permission is required if the information
requested will be tied to the child's ATI Communication Preference
Center Profile, or if there is a possibility that personal
information may be disclosed by the user to others. To help
protect identities, we encourage participants in the community
areas to use their screen names instead of their given name,
username or e-mail address. Please be aware that parental permission
is not required for participation if the information collected
is not tied to the child's ATI Communication Preference Center
Profile, or if there is no chance for personal information
to be disclosed to others.
ATI Technologies Inc. respects the privacy rights of our online
